On PS CS5, on top of the screen, there is the menu choice (File, Edit, Image, Layer, etc.). To the right of it, sometimes it display some icons like Br, MB, ViewExtra, Zoom Level, ArrangeDocument and ScreenMode. Sometimes, for some unknown reason, this icons do not show up.

Is there a way to turn it on and off?

I am pretty sure that what you are talking about is tool dependent. As you change your tools, your options at the top will change to match the tool you are using.

No, it is not the tool dependent interface.

I think it is a screen refresh issue. Sometimes it doesn't show up, but if I mouse of the proximity, it seems that it's there.

OK, interesting.... Just one thought; do you have the newest software version for your video card? I remember a while ago that some funky things were happening when I zoomed in until I updated the video card drivers. Not sure if it would be related, but might be worth looking at.

I am pretty sure I have the new version of the video driver. I did that when I calibrated my monitors with my Spyder.

Here is a screen shot of what is missing. The circled red portion sometimes doesn't display. But there is a space there for it. When I moused over it, I can see there is something there (No. It doesn't reflresh it).

That seems to be a toolbar issue, can you right click on it to see if you have options?

Are you using a Mac or PC? It would be easier to figure out if you had a screen shot if it when it's not displaying properly. The area you have circled is application bar and it has the workspace switcher and other application controls. You can change this and hide some items from the Window menu. Check your workspace and make sure you don't have something checked out of the ordinary for you. Or try to make it happen so you can see what it is that you are doing to cause it.

it could be that you have pressed the tab key. This toggles all the tool bars and blocks on and off, very useful while working on an image as it clearst he screen of all the clutter and gives a larger working area. My tab key needs a replacement spring from serious overuse.
